Senior Assessment Center
Sometimes, seniors face difficult changes as they age leaving family members concerned about their well-being. The Senior Assessment Center at Coastal Communities Hospital is specially geared toward the health concerns of seniors and helping them remain as active and independent as possible.
The Senior Assessment Center is a comprehensive outpatient evaluation and treatment service for seniors who are experiencing difficulty managing at home. Problems may include:
At the patient's first office visit our team of senior care specialists will perform an initial assessment that may include:
- dramatic weight loss
- pressure sores
- urinary incontinence
- chronic illness
- chronic pain
- physical weakness
- Alzheimer's Disease
- a comprehensive physical
- functional and psychosocial evaluation
- treatment planning by our interdisciplinary staff
Our team carefully evaluates each patient and prepares a written summary of their findings and recommendations. The Center may provide ongoing care or make referrals to primary care physicians and specialists in the community.
As an additional resource for Alzheimer's patients and their family, the Alzheimer's Association has affiliated with Coastal's Senior Assessment Center and is now located on-site for your convenience.
The Senior Assessment Center is conveniently located within the hospital and is open weekdays from 8:30 a.m. to 4:30 p.m. and by appointment.
Coastal Communities Hospital and the Senior Assessment Center are reaching out to the community by sponsoring health related and Alzheimer's events. We welcome you to join us at one of our Workshops or at one the Alzheimer's support groups being held at Coastal Communities Hospital.
